dc.descriptionRespiratory failure is a serious condition that impedes autonomous breathing. This condition arises when the lungs fail to provide sufficient oxygen to the blood. It occurs rapidly, often without warning, and is frequently caused by diseases or injuries that affect breathing, such as pneumonia. In Ecuador, in 2022, the most affected age group was 1 to 4 years, followed by 65 years and older. The highest incidence was recorded in males with 2,083 cases. The present case study aimed to determine nursing interventions in geriatric patients with acute respiratory failure secondary to unspecified pneumonia, for the rehabilitation and improvement of their health condition. Based on the analysis of the clinical history, the Marjory Gordon functional patterns, the Barthel index, and the Norton scale were established as tools. Finally, the factors that influenced the patient's evolution were determined, the most important being age over 65 years. After analyzing the comprehensive nursing assessment, based on the Marjory Gordon functional patterns, alterations were identified in the following patterns: health perception and management, nutritional metabolism, and activity/exercise; therefore, the following care plans were designed: impaired spontaneous ventilation, pressure injury in adults, impaired physical mobility, and ineffective self-management of health. In addition, the Barthel index tool was assessed, obtaining a total score of 30 points, which indicated a degree of severe dependence for performing activities of daily living.
dc.description.abstractLa insuficiencia respiratoria es una condición grave que impide la respiración autónoma, esta afección surge cuando los pulmones no logran proporcionar suficiente oxígeno a la sangre, presentándose de manera rápida, a menudo sin previo aviso, siendo frecuentemente causada por enfermedades o lesiones que afectan la respiración, como la neumonía. En Ecuador, en el 2022 el grupo de edad que resultó más afectado fue el de 1 a 4 años, seguido por el de 65 años y más, la mayor incidencia se registró en el género masculino con 2.083 casos. El presente estudio de caso tuvo como objetivo determinar las intervenciones de enfermería en el paciente geriátrico con insuficiencia respiratoria aguda secundaria a neumonía no especificada, para la rehabilitación y mejoramiento de su condición de salud. De acuerdo al análisis del historial clínico se establecieron como herramientas, los patrones funcionales de Marjory Gordon, el índice de Barthel y la escala de Norton. Finalmente, se determinaron los factores que influyeron en la evolución del paciente siendo el principal la edad mayor a 65 años, tras el análisis de la valoración integral de enfermería, basado en los patrones funcionales de Marjory Gordon, se identificaron alteraciones en los siguientes patrones: percepción y manejo de la salud, nutricional metabólico, y actividad ejercicio; por lo que se diseñaron los siguientes planes de cuidados: ventilación espontánea deteriorada, lesión por presión en adultos, movilidad física deteriorada y autogestión ineficaz de la salud. Además, se valoró la herramienta del índice de Barthel obteniendo un puntaje total de 30 puntos, lo que indicó un grado de dependencia severa para la realización de las actividades de la vida diaria.
